The show was pitched as 'Do You Remember Grade School?' in November 2006 by Burnett and Zoo Productions; Fox's Peter Liguori was the only network president able to win the six-question quiz. Fox announced the pickup on January 31, 2007, and Jeff Foxworthy was named host on February 9. The first episode aired less than eight weeks after the pitch.

A single contestant answers ten questions (plus a final bonus question) drawn from elementary school textbooks, two from each grade level first through fifth. Contestants can use three cheats—Peek, Copy, and Save—assisted by a 'classmate' from a cast of five school-age children. If they answer incorrectly, drop out, or fail to win the top prize, they must state they are 'not smarter than a 5th grader.' Two people won the $1 million prize: Kathy Cox, Georgia superintendent of schools, and George Smoot, Nobel Prize-winning physicist. Two won $250,000 in syndication, and one won $100,000 on Nickelodeon.

The show was revived on Fox in 2015 with a new generation of fifth-graders and a sixth-grade million-dollar question. It later aired on Nickelodeon in 2019 with host John Cena. A separate spin-off, 'Are You Smarter Than a Celebrity?', premiered on Amazon Prime Video in October 2024, hosted by Travis Kelce, featuring celebrity contestants.
